I believe Zbrush is made to function best on a mesh with a size around 1. You definitely need too customize your brush size but in general I've pointed out a critical issue that would resolve this by default and that is that CC4 needs to activate Unify to scale it to Zbrush unit space to work properly with symmetry and other tools related otherwise it looses symmetry!! Are you sure? In the Screenshot, you can see that changing the brush size slider to from the default 64, doesn't allow you a large enough brush to do much on the mesh.
